Insurance, Physical Exam & Sports Information Forms
All students who plan on participating on any intercollegiate athletic team at St. Norbert College must have completed medical forms and insurance information on file in the athletic training room office before being allowed to participate in any practice or contest. They also must have completed the online sports information form.All forms are in PDF format. If you are unable to print these forms as they appear on the screen, please contact the St. Norbert College Training Room at 920-403-3179.
Incoming Freshmen and Transfer Students
You must download, complete and return the following three forms (six pages plus online form) prior to your first participation:
- Physical exam forms (3 pages) - You must use these forms for your physical exam. We will not accept any exam from your senior year in high school or earlier.
- Insurance forms (3 pages)
- Sports information questionnaire

Important note: These forms are completely separate and should be treated separately from any forms required by the St. Norbert College Health and Wellness Center. Do not address or fax any forms with the heading "Intercollegiate Athletics" to the Health Center. Do not address or fax any forms with the heading "Health Services" to the Athletic Department.
