Lost, Stolen or Damaged Cards

Reasons for Replacing a Damaged Card

  • “Misuse”:  ($15 charge) If your card is damaged due to neglect/abuse (ie. card is bent due to placing it unprotected in a pocket, biting on it, excessive scratching from being dropped on the ground or punching un-needed holes in card.)

What Should I Do if My Card Gets Lost or Stolen?
If your card has been lost or stolen, follow the steps below:

Step One

Call Campus Card Services at 920-403-4030 Monday through Friday, between 8 a.m.-4:30 p.m. (After hours, call Campus Safety at 920-403-3299 to report your SNC ID card lost and request a block be placed on it.) This will prevent anyone else from using your card until you find it or have a replacement made. By reporting your card lost and putting a block on it you can waive the responsibility of any lost funds thereafter. If you neglect to have your card blocked, you will be responsible for any purchases made on the card until you inform us of your loss.

Note: You may call Campus Safety to have a temporary ID issued to you. There is no charge for a temporary card as long as it is returned within seven days of being issued, otherwise a charge of $5 will be added to your account. To obtain a temporary card, contact Campus Safety at 920-403-3299. Tell them you have lost your SNC ID card and need a temporary card. Campus Safety will meet with you at their earliest opportunity and issue a temporary card. Please bring another form of ID with you. The temporary card is good for seven days from the date of issue. After that period, you will be responsible for coming into campus card services for a new card. If the seven days have elapsed, we will turn off the usage of your card. You will need to bring the temp card back to Campus Card Services to reactivate your account.

Step Two

Come to Campus Card Services to have your card replaced. The replacement fee for your ID card is $15.00.

Note:
 If your SNC ID card was stolen and the individual responsible for the theft is identified, they will be charged the replacement fee.

If you are replacing a damaged SNC ID card, there is a $15 charge (depending on the problem).
