Health and Wellness Services Requirements and Policies

All New and Transfer students must complete the online health, immunization and insurance forms.
Go to https://stnorbert.medicatconnect.com enter your SNC username and password and follow the instructions to complete these forms.

REQUIRED: Two (2) measles, mumps, and rubella (MMR) immunizations given after the first birthday.

Provide all dates of immunizations you have received both as a child and as an adult. Complete immunization records are most important in meeting your health needs while you are at St. Norbert College.

You may waive out of these required immunizations by completing the conviction waiver, however, this may result in your quarantine if a vaccine preventable disease develops on campus.

Wisconsin residents may find their immunization records in the Wisconsin Immunization Registry (WIR) http://dhfswir.org

REQUIRED: Unless you provide proof of health insurance, you will be enrolled in the St. Norbert College/ WAICU Health Insurance plan. The informational brochure and enrollment forms are available for download www.snc.edu/health/fees.html and follow links.

Chronic Medical/Mental Condition

A Plan of Care is required for ongoing medical or mental health conditions. Please send completed plan to Health and Wellness Services or fax to our office at the address or fax no. listed. 

Medical Documentation for Readmission

Students with health conditions who cannot complete their coursework AND the last date to withdraw from courses has passed, may be eligible for a Medical Withdrawal. 

Students who will be competing in intercollegiate sports must complete specific health forms located on the athletics department website www.snc.edu/athletics
