Health and Wellness Services policies
Enrollment policy
The following documents are required for enrollment at St. Norbert College.
1.
A health history/immunization record and a
tuberculosis questionnaire
Both of these documents must be on file in Health and Wellness Services to complete registration for classes. A medical exam and plan of care are only required if a chronic medical condition exists (diabetes, epilepsy, eating disorder, physical disability, etc.).
2. Immunizations
Each student's immunizations must be documented by the date of entrance into St. Norbert College. These include:
- 2 MMR; 1st dose after the first birthday, 2nd dose at least 1 month after first dose
- Diphtheria, Tetanus, Pertussis (4) DTP
- Diphtheria, Tetanus (every 10 years) Td
- Tetanus/diphtheria/acellular pertussis one time dose after age 8; Dtap or Tdap
Immunizations that are not up to date may be obtained at your doctor's office or at St. Norbert College Health and Wellness Services. Students who do not provide dates of immunizations will be prevented from registering for classes until proof of record is received.
If you find it difficult to locate your immunization records, contact your pediatrician's office, your high school or grade school, or the local public health office. Please make every effort to have these records sent to us prior to enrollment in classes. It becomes increasingly difficult to track down records after you arrive on campus.
3. Personal Health Insurance
Personal Health Insurance is a requirement to attend St. Norbert College. Students will automatically be enrolled in the
St. Norbert College Student Health Insurance Plan, unless an
insurance waiver form is submitted.
Please review with your insurance company where health care can be accessed in the Green Bay area. You may wish to send a
medical access form to St. Norbert College Health and Wellness Services. A medical access form will allow Health and Wellness Services to more easily make referrals to specified area clinics with less stress for students in determining where they can access medical care.
Special note: A TB skin test within the past year is required for all foreign born students and individuals who lived in another country for more than three months.

Medical withdrawal policy
For students with health conditions who cannot complete their course work and the last date to withdraw from courses has expired, they may be eligible for a
medical withdrawal.
