Event Form
This registration process is designed to help you plan your
event and reduce the liability risks inherent at an off-campus
function, whether it be a field trip to a local business on
Wednesday afternoon, or a dinner dance on Friday night.
Registration indicates the college is aware you have taken proper
precautions and planned accordingly based on information provided
on this form.
The form goes directly to the Department of Leadership, Service
& Involvement upon submission. You will receive an email back
from your LSI liaison within 2 business days either completing your
registration as you have planned, or requesting further action
(i.e., a consultation meeting). It also goes to Kristee Becker in
Risk Management. She will contact you regarding any waivers you may
need to complete for the said event.
Upon submission, copies of this email will also be forwarded to
the organization's advisor and president. You will receive a copy
back per email upon completed registration, or instructions for
further action, by your organization's LSI liaison.
