This form is used to help advise you on course selection before you have sent a final transcript of your current courses to SNC. This information will automatically be forwarded to the Registrar's and Advisement Offices. Enter your name, SNC ID (if known), and Email, and enter the institution you are attending. For each course enrolled list the instititional course number (Subject and Course Number) and course title. Depending on the school you are attending enter the credits, points, courses, or hours of your courses if know. Any comments you want to add may be entered in the right hand column.
