Insurance Claims

Incident Reporting Instructions
1. Call 911 whenever incident involves:
  • Serious injury and/or illness to person(s)
  • A serious event such as fire, or 
  • Whenever involved in an auto accident. 
2. ALL incidents involving property damage, personal injuries and/or automobile accidents must be reported to Campus Safety at 920-403-3260 to ensure an official college incident report has been filed.

Employee Injury (Worker’s Compensation)
All employee injuries must be reported to Campus Safety at 920-403-3260 and then you must fill out a First Report of Injury or Illness form. Upon completing the form, it must be submitted to and completed by your supervisor, then returned to Human Resources. Human Resources must receive the completed form within five calendar days, including weekends.

Damage or Theft of College-Owned Property
After reporting the incident to Campus Safety, email or call the director of risk and property management at dave.nalepka@snc.edu or 920-403-3066.

Vehicle Accidents (St. Norbert College-Owned or-Rented for Business Purposes)
After reporting the incident to local law enforcement and Campus Safety, contact the director of risk and property management at 920-403-3066 or dave.nalepka@snc.edu so an official insurance claim can be initiated. Please provide the police report or any information obtained from the local law enforcement agency that responded to the call.
