The annual evaluation report is not a form as it has been in the past. This report is compiled and presented by each organization as they desire, highlighting the aspects of the Green Knights' elements they have accomplished. While accomplishing all aspects is required to achieve all 5 Knights status, completing what you can is also important. all annual evaluation reports, big and small are due February 15 in the Department of Leadership, Service & Involvement. The report should encompass the prior year (past spring and fall semesters). Following is an example of what would be in an annual report if all elements were completed:
Cover page summarizing contents of report and highlighting the year - no more than 1 page
Event Attendance Registration for Leadership Development
Copy of Organization's web site
Event Attendance Registration for each organization leadership/education program /workshop
Copy of year end, finalized, budget in good standing
Copies of each off campus event email registration
Copies of public relations articles and pictures regarding your organization's endeavors
Event Attendance Registration for each education event attended
Copy of program co-sponsorship agreement(s)/publicity
Copy of alumni communication
Copy of Community Partnership agreement/publicity
National affiliation report or agreement
It is also important to note there are forms due apart from the annual report. These include:
