All off campus event forms are filled out a minimum of 10 days prior to every scheduled event held off campus.
Attach a copy of all off-campus event request emails to the annual evaluation report.

Submit all documentation as requested by Leadership, Service & Involvement, on time and complete.

25 Points • Policy Sign Off Form
Submit the completed
Policy Sign Off form at the fall access meeting and hazing, alcohol, and non-discrimination policies are incorporated into the organization's constitution.
