Early Alert

In case of emergency, call 911, 988 (Suicide and Crisis hotline) or Campus Safety at 920-403-3299. Please note that this website is not monitored 24/7. 


Our goal is to provide students, faculty, staff and visitors a safe environment in which to live, study and work. You are a partner in this mission! In alignment with the college’s Catholic and Norbertine values, we approach each report with great respect for the dignity of the individual.

Using the forms below, please describe the behaviors of concern and any related information. If you choose to give us your name, we may contact you if we have further questions. Any information you provide in this referral will be maintained with the strictest privacy. However, the information may be shared with appropriate college staff who can best respond to the concern and under the restrictions set forth by FERPA.

Concerns Regarding Students

Use this Early Alert reporting form to share concerns about:

  • The health or welfare of a student
  • Attendance or academic concerns
  • Discrimination based on identity
  • Report the loss of a friend or family member
  • Behavioral concerns

This report form is only for concerns regarding students.  The link below will take you to a page where you will first search the name of the student you are concerned about, and then a form will be provided for you to share your concern regarding that student.


Academic Misconduct

Submit the academic misconduct reporting form for:

  • Cheating
  • Plagiarism
  • Other academic integrity concerns


Sex and Gender Harassment/Discrimination

Submit the Sex and Gender Harassment/Discrimination reporting form for:

  • Sexual Assault
  • Sexual Harassment
  • Fondling
  • Dating/Domestic violence
  • Stalking
A report filed online will go the the Title IX coordinator. Please note that unless remaining anonymous, the identity of the individual(s) involved is obtained for the report but only shared with investigation personnel and those who need to know.



File a Grievance Regarding Faculty/Staff

Use this only for complaints regarding faculty/staff or school issues!  If your concern is regarding a student - use the Early Alert form.

The U.S. Department of Education’s Program Integrity Rule requires that each college or university authorized to offer postsecondary education in one or more states ensures access to a complaint process that will permit student consumers to address the following:

  • Alleged violations of state consumer protection laws that include but are not limited to fraud and false advertising;
  • Alleged violations of state laws or rules relating to the licensure of postsecondary institutions; and
  • Complaints relating to the quality of education or other state or accreditation requirements.
